Consumer Health Data Privacy Policy

Your health information,

What we collect when you take the HormoneIQ™ assessment, why, who receives it, and how to see or delete it.

Effective November 3, 2026

The short version

I.Who we are

This policy covers the HormoneIQ™ assessment and related pages at quiz.drnatasharyan.com, and drnatasharyan.com, run by Dr. Natasha Ryan, ND ("we," "us"). It explains how we handle "consumer health data" as defined by Washington's My Health My Data Act and similar state laws, including those in Nevada and Connecticut.

The assessment is educational. It does not diagnose any condition and does not create a doctor–patient relationship. If you later become a patient, your patient records are handled separately under our practice's patient privacy notice.

II.What we collect

We collect this directly from you. We do not buy health information about you from anyone.

III.Why we collect it

We do not use your health data for any other purpose without asking you first.

IV.Who receives it

We use a small number of service providers to run the assessment. They handle your information only to provide their service to us.

ProviderWhat they doWhat they receive
FormspreeReceives and stores resultsName, email, age range, scores, tier, archetypes

We do not sell your consumer health data. We do not share it with advertising networks or social media platforms, and we do not use it to target ads.

V.Your rights

You can, at any time:

To use any of these rights, email us at the address below with the subject line "Health Data Request." To protect your privacy, we may confirm your identity, usually by asking you to write from the email address you used for the assessment. We will respond within 45 days. We will never charge you or treat you differently for using these rights.

Appeals. If we decline your request, you can appeal by replying to our decision with the word "Appeal." We will respond within 45 days and explain our decision. If you are still not satisfied, you can contact your state Attorney General (in Washington: atg.wa.gov/file-complaint).

VI.How long we keep it, and how we protect it

We keep your results while you remain on our email list, and for up to two years after your last contact with us, unless you ask us to delete them sooner. Access is limited to Dr. Ryan and the providers listed above. Data is encrypted in transit, and our accounts are protected with strong passwords and two-step sign-in.

VII.Changes to this policy

If we change how we collect or share health data, we will update this page and ask for your consent again before using your data in a new way.
